When choosing a primary care physician, a person may only choose one general practitioner, paediatrician, gynaecologist and dentist.

As a rule, a physician is chosen based on the place of residence, in the nearest healthcare institution.

By using the search engine, you can find the information on physicians who have a contract with the Croatian Health Insurance Fund, as well as the information about which physicians are accepting new patients.
At the office of the physician you have chosen, the physician will fill in the statement form with the relevant data and certify it by affixing their signature and the seal of the healthcare institution, i.e. their office, onto it.
You should then submit this certified form to the competent office of the Croatian Health Insurance Fund in your place of temporary stay/permanent residence.
